
Know How To Go South Central CT is the mobility management program for the 32 towns of the south central region. Mobility management is an innovative, customer-focused approach for managing and delivering coordinated transportation services to older adults, people with disabilities and people with low-income. Mobility management focuses on meeting individual customer needs through a wide range of transportation options and service providers. It also focuses on coordinating these services and providers to achieve a more efficient transportation service delivery system.

Reduce Fare ID Application
Connecticut bus and rail systems offer a reduced fare fare for seniors 65 and over and people with disabilities. To qualify, you must present a Medicare card or the Connecticut Statewide Reduced Fare ID card. The application form for the ID card can be found online. The mobility manager can provide assistance to those who need it, including help with obtaining the required passport size photo.

Connecticut Non-Emergency Medical Transportation (NEMT)
NEMT is available to eligible Medicaid members in Connecticut who have no other way of getting to their medical, behavioral health, or dental appointments at no cost. Call 1-855-478-7350 Monday through Friday from 7 a.m. to 6 p.m. to learn more an to schedule. You must schedule your trip at least two business days before your appointment, unless your trip is urgent. Urgent rides are scheduled 24 hours a day, seven days a week.

Disabled American Veterans (DAV)
DAV operates a fleet of vehicles around the country to provide free transportation to VA medical facilities for injured and ill veterans. DAV stepped in to help veterans get the care they need when the federal government terminated its program that helped many of them pay for transportation to and from medical facilities. The vans are driven by volunteers, and the rides coordinated by almost 192 Hospital Service Coordinators around the country. Canoe Brook Center has three busses, two 18 passenger busses and one 12 passenger bus that are driven by CDL licensed bus drivers employed part-time by the Town of Branford.
Passengers must be Branford residents age 60+, current Canoe Brook members, orient-ed, ambulatory and independent. Our bus-ses have three steps that must be climbed to enter the bus; you must be able to enter the vehicle unassisted. Canoe Brook membership is $8 a year.
Service is curb-to-curb. Should you need hands on assistance or wheelchair transpor-tation call Greater New Haven Transit District’s MyRide at 288-6282 as they provide wheelchair transport to qualifying Branford residents.

Branford Medical Transportation
Medical Transportation is available to Branford residents 21 years of age or older. The program offers 3 Subaru Forester cars that are provided by the Town of Branford and a team of dedicated volunteer drivers. This service is curb to curb. You must be able to get in and out of the car unassisted. All new passengers will be evaluated prior to scheduling a ride. For safety reasons, they are unable to transport any passengers who have undergone a procedure with any kind of sedation.
Transportation is provided to doctors’ offices in Branford, East Haven, New Haven, Guilford, Madison, and the VA.
The earliest appointment is 9:00 a.m. and last pick up from a doctor’s office is 2:30 p.m. All cars are off the road by 3:00pm

We provide various transportation services for senior and disabled Cromwell residents, including dial-a-ride, shopping routes, transportation to senior center programs, and outtrips, transportation service is a first come, first serve service. In order to help us serve you better, the following tips may be useful.
Medical appointments are our priority. Transportation for Medical appointments may be requested up to two months before the appointment. We transport for both local and out-of-district appointments. We consider local appointments to be Cromwell, Middletown, and Rocky Hill. Out-of-district appointments could include Hartford, West Hartford, New Britain, Meriden, Glastonbury, or Farmington. These out-of-district appointments must be made at least two weeks ahead of time and can be done only if our appointment schedule allows.
Requests for transportation to Senior Center programs, Outtrips, or for our twice weekly grocery shopping route, must be made at least 24 hours in advance. This service is just for Cromwell residents and is no cost.

All passengers must be registered as members, with current emergency contact information on file. Our Senior Transportation Services provide rides for the following essential services: to/from the East Hampton Senior Center, medical appointments in town and pharmacy visits.

FISH provides free transportation to and from medical, dental and physical therapy appointments in the area roughly bounded by Middletown, New London and Guilford. The service is available Monday through Friday from 8 am to 5 pm for ambulatory residents of Essex, Deep River and Chester who have no other means of transportation. The Dial-A-Ride Transportation Program is administered and operated by the Madison Senior Center. Rides are given in a 14-town region for medical appointments, and personal business on a first come first served basis.
Eligible Riders
Madison residents; age 60 or older or disabled Madison residents over the age 18 having certification of a medical disability or disabled are eligible to use Dial-A-Ride. Persons with medical restrictions need to submit proof of disability to the Senior Center. Those not sufficiently alert, ambulatory, or a in wheelchair need to be accompanied by an aide. New Riders are required to file emergency contact information with the Senior Center.
Priority Use of Rides
Medical appointments take precedence with scheduling, remaining trips are scheduled on a first-come, first-served basis. We reserve the right to refer passengers to other services and to limit the number of rides an individual may receive to best serve the majority.

Please call the Mini-Bus office at 203-237-3338 to schedule your ride. The application for our transportation program, in English and Spanish, can be found by clicking this link. Hard copies are available at the Senior Center.
Rides are available for Meriden residents age 55 and over and for people with disabilities under age 55. Applications are available in the Mini-Bus Office.
Rides: We provide rides to go shopping, to the bank, the pharmacy, hair salon, convalescent home visiting and to and from the Senior Center. To make a reservation, please call by 12:30pm the day before your ride. Due to full rider demand, if your ride is not pre-scheduled there will be no additional scheduling for that day. Only trips on the schedule will be made; we cannot bring you anywhere other than your scheduled appointment. If you need to cancel your reservation please call the Mini-Bus office starting at 8:00am.

The mission of Senior Transportation Services, Inc. (STS)

STS volunteer drivers use their own vehicles to provide curb to curb, safe, reliable and economical medical transportation to out of town appointments, pharmacy and care facilities to visit loved ones.

Orange Senior Center Transportation
The Orange Senior Center provides transportation to Orange residents, age 55+, and/or disabled residents. Caregivers are welcome to accompany riders. All buses are equipped with a wheelchair lift. Service animals and assistive devices welcome. Please be aware that the drivers do not provide assistance to and from the vehicle.
Riders need to submit an application and complete a home visit with the Municipal Agent in order to be approved to participate in the program. If staff determines that transportation is not appropriate, the resident is referred to alternative transportation.

A driver will pick you up in a passenger vehicle at your home, take you to your appointment, and bring you back home after. Appointments are taken on a first-come, first-serve basis, as we are limited by the number of passenger vehicles and drivers available. We do our best to accommodate. Fees: $35 within 9 town area; $65 to Guilford, Madison and Middletown; $100 for out of area destinations up to 5 hours ($20 an hour over 5). Utilizing a fixed route bus is one of the most affordable ways to access the community, with fares as low as $.85 per trip. The District provides ADA paratransit service for Woodbridge. Limited dial-a-ride service is provided to Guilford. Transportation is offered seven days a week during the same hours as the fixed route bus operates. For information call 203-288-6282; for paratransit service call 203-288-6643. The cost for a one-way ADA (Americans with Disabilities Act) paratransit ride with Greater New Haven Transit District (GNHTD) is $3.50. GNHTD drivers cannot make change, so exact fare is required. You can also purchase ticket books containing ten tickets at a discounted price of $28.00

Right-away service requests accepted, advanced reservations accepted up to 7 days in advance. Passenger pays only $5 for each trip up to 8 miles, $2.90 for each mile over 8 miles.
