Know How To Go South Central CT is the mobility management program for the 32 towns of the south central region.  Mobility management is an innovative, customer-focused approach for managing and delivering coordinated transportation services to older adults, people with disabilities and people with low-income. Mobility management focuses on meeting individual customer needs through a wide range of transportation options and service providers. It also focuses on coordinating these services and providers to achieve a more efficient transportation service delivery system.

Mobility management involves:

  • Developing an inventory of available services

  • Identifying customer needs

  • Developing strategies to meet customer needs

  • Coordinating financial and other resources

  • Improving coordination through transportation brokerage systems

  • Promoting the use of innovative technologies, services, and other methods to improve customer service and coordination

  • Developing customer information and trip planning systems

Mobility managers serve as policy coordinators, operations service brokers, and customer travel navigators. They coordinate transportation services among all customer groups, service providers, and funding agencies and work with human service agencies and workforce centers that coordinate the travel and trip planning needs of individuals who receive human service program assistance.

Transportation Financial Resources

  • Reduced Fare for Bus & Rail

    Connecticut bus and rail systems offer a reduced fare fare for seniors 65 and over and people with disabilities.  To qualify, you must present a Medicare card or the Connecticut Statewide Reduced Fare ID card.  The application form for the ID card can be found online.  The mobility manager can provide assistance to those who need it, including help with obtaining the required passport size photo.

  • Senior Resources Daisy Fund

Statewide Transportation Resources

  • ALS Association

    The ALS Association Connecticut Chapter (ALSA CT) provides curb to curb livery service and van rentals. Participants must be registered with ALSA CT and have no other alternative service available.   • American Cancer Society

    The American Cancer Society Road To Recovery® program eases your burden by giving free rides to cancer-related medical appointments. Trained volunteer drivers pick you up, take you to your appointment, and drop you off at home. Patients must be traveling to a cancer-related medical appointment. Other eligibility requirements may apply.  Call 1-800-227-2345 to learn more.

  • Connecticut Non-Emergency Medical Transportation (NEMT)

    NEMT is available to eligible Medicaid members in Connecticut who have no other way of getting to their medical, behavioral health, or dental appointments at no cost.  Call 1-855-478-7350 Monday through Friday from 7 a.m. to 6 p.m. to learn more an to schedule. You must schedule your trip at least two business days before your appointment, unless your trip is urgent. Urgent rides are scheduled 24 hours a day, seven days a week.

  • Disabled American Veterans (DAV)

    DAV operates a fleet of vehicles around the country to provide free transportation to VA medical facilities for injured and ill veterans. DAV stepped in to help veterans get the care they need when the federal government terminated its program that helped many of them pay for transportation to and from medical facilities. The vans are driven by volunteers, and the rides coordinated by almost 192 Hospital Service Coordinators around the country. Provides rides for only the following purposes: medical appointments, or grocery shopping. Also, you must meet at least one of these criteria: you have a disability. No membership is required. The Transportation Assistance Program for Older Adults offers affordable transportation services at $3.00 per destination, or $2.00 for one-way trips within the Town of Bethany. This convenient program utilizes buses or trains to provide older adults with transportation options to reach their desired destinations.

Right-away service requests accepted, advanced reservations accepted up to 7 days in advance. Passenger pays only $5 for each trip up to 8 miles, $2.90 for each mile over 8 miles.
